PATIENT INFORMATION

Dr. Braunlich collaborates with other providers in the field of physical therapy, as well as medical equipment companies that supply components for total joints, fractures and sports medicine procedures. Listed are some of our most utilized and recommended collaborations.

Patients are provided with the official Braunlich Orthopedic patient registration form and patient history form. Likewise, referring physicians have access to the Braunlich Orthopedic referral form.

The documents listed provide post-operative instructions for patients upon discharge. Patients may access the appropriate PDF file for information otherwise instructed by the physician. Patients may download files to provide themselves with a printed copy.
